Site Evaluation Summary

For my first history and physical I wrote about a patient I saw in the Emergency Department as a surgery consult. I selected this case because I saw him from the ED to the operating room and post-operatively. This patient presented to the ED with irreducible inguinal hernia that required same–day operative management. In the ED we were able to reduce the hernia and discovered that it was not strangulated. This was my first time writing a consult note and I learned a lot. My note was disorganized, as I wrote it as an ED note, a consult note, and a post operative planning note. I learned that for consult notes focus write about what was done in the ED, what we did as the consult service, and what we plan to do.

For my second H&P I wanted to do another Emergency Room consult in order to learn from my previous mistake, but none of the other consults I had would provide a robust differential diagnosis. I decided to write about a 33 y/o female with unilateral nipple discharge. I selected this case because I wanted to ensure that I knew the differential diagnosis, workup, and management of nipple discharge.
