I feel that my note taking skills strengthened over the course of this rotation.
For my first H&P I presented a case on a 25 y/o female with palpitations with some associated chest pain and dyspnea on exertion. I feel that I did a good job creating a differential diagnosis in my head and guiding my questions based on that differential diagnosis. However, one area of improvement was the workup for my patient. One differential diagnosis I had was anemia for which I ordered a CBC; however, I should have also ordered iron studies as sometimes people can have a normal CBC with abnormal iron studies.
For my second H&P, I presented a 67 y/o female with bilateral neck masses for two months, which on exam was bilateral tonsillar lymphadenopathy. I wrote an additional H&P about a 63 y/o female with right hand pain, swelling, and redness. Although I did not present this case, my site evaluator went over my case in our meeting, as it presented many learning opportunities. I learned more about documenting hand complaints, such as including hand dominance as well as whether the area of erythema was circumferential or non-circumferential. I feel that when it comes to skin findings, I have some trouble describing what I find on examination, and that this discussion was extremely helpful for me going forward. I also learned the importance of asking if the patient has had a recent manicure, as this could be a potential source of infection.
